ENTRY FEE: $400 PER TEAM
Deadline for Registration: September 3, 2010
To register a team, please fill out and submit the form below.
You can list up to twelve (12) players.

**Cash, Certified Checks, or Money Oders must be received before registration is considered "official".**
PLEASE NOTE: Entry fee must be received at least 48 hours prior to tip off of first tournament game.
If your payment has not been received before this time period, your team will be eliminated from ALL tournament brackets.

Each team must print out the Basketball Release Form, and have each team member sign and date it. The form must be submitted to the Hoop It Up Staff prior to the team taking the floor. IF ANY MEMBER DOES NOT SIGN, THAT INDIVIDUAL WILL NOT BE ABLE TO PARTICIPATE IN THE GAME(S) UNTIL THE FORM IS SIGNED.
